A student shall show acceptable evidence of vaccination prior to entry or transfer to a public elementary or secondary school in Texas. Any time your child receives a vaccine, please bring your child's shot record to school so his/her health record can be updated. You may also email the nurse a clear picture of the shot record that includes the student’s name and date of birth. We will send you a courtesy notice if one of your child’s immunizations comes due during the school year. Please keep in mind when transferring from another school to request a copy of your vaccination record before entry if you do not have it. Only the parent/legal guardian can request a copy of your child's immunization record and request must be in writing to the school nurse.
